Tribute Gifts 
Recognize the people who have made a difference in your life with a tribute gift. When you make a contribution, a personalized card will be sent notifying the honoree of your thoughtfulness.  We ask for a minimum of $25 donation for our holiday cards during November and December.  To make a gift in someone's honor, print this form and mail with your contribution or make a donation online and complete this tribute form.  To have tribute cards sent to multiple recipients, please contact Wanda Couts at (205) 327-3810.

Online Gifts
